WHO WE SERVE: PATIENTS

You've been told your condition is something to manage. We're here to challenge that.

Most of the healthcare system is built around managing chronic disease. Nudj is built around changing it.
But before we treat a single condition, we start somewhere different. We start with you: who you are, what matters to you, and what a healthy life actually looks like in the context of your life. That understanding is what the entire clinical program is built around.

Then comes the clinical work. A physician-led care team, delivered from your home, working alongside the doctor you already have, using six evidence-based pillars of lifestyle medicine to address the root causes of your condition and help you change the trajectory of your health. Not in years. In 16 weeks.

If the system has ever made you feel like a chart instead of a person, this is different.

Nearly one in five adults has felt dismissed, not heard, or not taken seriously by a physician. If that has been your experience, you may have stopped asking for help, stopped expecting things to change, or stopped believing the healthcare system has anything different to offer you.

Nudj starts somewhere different. Before your first appointment, before your care team reviews a single lab result, we ask you something most healthcare programs never ask: what matters to you? What do you want your health to make possible? What would change if you felt better?

Your answers shape everything that follows. The clinical work, the care team focus, the milestones we set, and the support you receive are all built around who you are and what you are working toward. Not around your diagnosis. Around you.

And if you have questions you have been too embarrassed to ask your doctor, conditions you have been afraid to bring up, or habits you are not proud of, this is a safe space for all of it. Your Nudj care team is not here to judge. They are here to help you move.

What Nudj Looks Like For You

Nudj is not a program you are enrolled in and then managed through. It is a program built around your life: your schedule, your barriers, your goals, and your deeper reasons for wanting to be healthier. The clinical work is rigorous and physician-supervised, and it starts from understanding you, not from a template.

1. A care team that comes to you

A physician, certified health coach, registered dietitian, fitness specialist, and behavioral health coordinator, all working together as one team, all focused on you and your specific health picture. You connect with them by telehealth from your home, on a schedule that fits your life. Monitoring devices are shipped directly to you. You do not need to travel, find a new facility, or rearrange your life to get care.

2. A program that works alongside your existing care

Nudj does not replace your doctor. It works alongside them. Your Nudj care team communicates with your physician, supports the care plan you already have in place, and keeps everyone in the loop at key milestones. You stay with the provider you trust. The relationship you have built with your doctor continues. Nudj extends what that relationship can do for you.

3. Sixteen weeks to clinically meaningful change

The Nudj program is structured across 16 weeks with clear milestones, so you know what progress looks like and when to expect it.

Week 4

80% of enrolled patients see measurable behavior change

Week 10

HbA1c, blood pressure, and weight begin to shift

Week 16

Meaningful clinical improvement, built to be sustainable

4. Addressing what medication alone cannot

Medication manages the symptoms of chronic disease. Lifestyle medicine addresses the root causes: nutrition, sleep, stress, movement, behavioral health, and social connection. These are the actual drivers of the conditions you are managing, and they are what the Nudj program targets directly, clinically, and consistently, alongside whatever your doctor has already prescribed. The two work together. Neither is replaced.

Available when you need it. Not just during office hours.

One of the hardest parts of managing a chronic condition is what happens between appointments. A symptom appears. A question comes up. Motivation dips on a Tuesday evening and there is no one to call. Most healthcare programs leave you in that gap.

Nudj is designed for the gap. Your care team is accessible between appointments, in the evenings, and on weekends, when life actually happens and support matters most. You are not alone between visits. You have a team.

This is not a hotline or a chatbot. It is your actual care team — the people who know your history, your goals, and your barriers — available to help you stay on track when staying on track is hardest.

Three Ways to Get Started

You do not need to have everything figured out before you take the first step. Whether your doctor referred you, your employer offers Nudj as a benefit, or you found this on your own, there is a path in from wherever you are starting.

Talk to Your Doctor

If you have a primary care physician or specialist you already trust, start there. Ask them about Nudj. They can refer you directly into the program, and your Nudj care team will communicate with them throughout, supporting the care plan you already have in place. You stay in the relationship. We extend what it can do.

Check With Your Employer or Health Plan

Nudj may already be available to you through your employer’s benefits package or your health plan’s covered programs. Contact your HR department, check your plan’s benefits portal, or call your member services line to find out whether you are eligible at no additional cost. Many people already enrolled in Nudj discovered it was available to them all along.

Request Enrollment Directly

If you are ready to get started now, you can request enrollment directly through Nudj. Our team will reach out to confirm your eligibility, walk you through how the program works, answer your questions, and connect you with your care team. You do not need a referral to start the conversation.

Your health. Your why. Your care team.

One physician-led team, built around who you are, delivered from your home, working alongside your existing doctor. Six evidence-based pillars of lifestyle medicine. Sixteen weeks to clinically meaningful change. The prescription is lifestyle. The foundation is you.
