Why the Model Exists
The healthcare system was built for acute care: appointments, prescriptions, procedures. It was not built for the sustained, between-visit, behavior-focused intervention that chronic disease requires. Nudj is built for exactly that — delivered at a frequency and continuity that visit-based care was never structured to provide.
It works for both sides. The physician extends their clinical reach without adding staff or changing how they practice. The patient receives sustained clinical support without leaving home or switching care teams. Friction removed from both sides of the equation.
What Defines the Model

Lifestyle Medicine-Infused Whole-Person Care
Not a wellness add-on. A structured, physician-supervised clinical care program model that integrates physical health, mental health, lifestyle behaviors, and upstream drivers of health, addressing the root causes of chronic disease through six interconnected, evidence-based pillars. The distinction matters because the outcomes are different: wellness programs produce engagement metrics; lifestyle medicine delivered clinically produces measurable change in HbA1c, blood pressure, medication burden, and total cost of care.

Collaborative, Not Standalone
Nudj works alongside the patient’s existing provider, not instead of them. Communication, coordination, and clinical updates flow between the Nudj team and the referring physician throughout the program. The patient stays in the care relationship they trust. The program extends what that relationship can do.

Interdisciplinary by Design
Physician, health coach, registered dietitian, clinical pharmacist, fitness specialist, behavioral health coordinator, and remote monitoring technician. Not separate vendors across separate platforms — one coordinated team working from the same understanding of the patient, accountable to one set of clinical outcomes.

Remote Delivery, Connected Monitoring
Devices shipped to the patient’s home. Telehealth touchpoints on the patient’s schedule, including evenings and weekends. Continuous monitoring with data surfaced to the care team in real time. No facility visits required.
The Six Pillars of Lifestyle Medicine
Every session, touchpoint, and intervention is grounded in one or more of the six pillars defined by the American College of Lifestyle Medicine. These are not six separate modules. They are six interconnected drivers of chronic disease, addressed simultaneously and calibrated to each patient.

PILLAR ONE
Healthy Eating
Nutrition plans built by a registered dietitian and calibrated to the patient’s conditions, metabolic markers, and life. Designed to reverse metabolic disease, not just manage it, with adjustments as clinical markers improve.

PILLAR TWO
Physical Activity
Movement protocols matched to cardiac and metabolic capacity, coached by a certified exercise specialist and tracked through connected devices. Progressively adjusted, and built around what the patient can actually sustain.

PILLAR THREE
Social Connection
Peer groups, care navigators, and family-inclusive coaching delivered through Nudj Village. Social isolation is one of the strongest modifiable risk factors for poor outcomes across every chronic condition. Social support is a clinical intervention, not a program amenity.

PILLAR FOUR
Stress Management
Guided breathing, mindfulness, and cognitive reframing embedded in every touchpoint rather than added as a separate module. Chronic stress elevates cortisol, drives blood glucose, increases cardiovascular risk, and disrupts sleep. Addressing it is clinical work.

PILLAR FIVE
Restorative Sleep
Personalized sleep protocols developed from each patient’s specific patterns and barriers. Sleep disruption worsens every cardiometabolic marker and undermines every other intervention in the program — which makes improving it one of the highest-leverage actions Nudj takes.

PILLAR SIX
Risk Reduction
Structured cessation support for tobacco, alcohol, and other substance use, delivered by the behavioral health coordinator inside the core program rather than as a separate referral. For patients managing chronic disease, risk reduction is a clinical priority.
The Nudj 16 Week Program
The structure is not arbitrary. It reflects the evidence base for meaningful, sustained behavior change in chronic disease populations.

Start
Onboarding, devices shipped, care team assigned, baseline clinical measurements established. The first conversation is about who the patient is and what they are working toward.

Baseline
First measurable behavior change documented. The care team reviews individual response and adjusts protocols. Most patients see their first proof the program is working at this milestone.

Inflection
Average HbA1c and blood pressure inflection in cardiometabolic patients. Behavior changes consolidating into sustainable patterns. Clinical markers shifting in ways that appear in lab results.

Outcome
Clinically meaningful outcomes documented, attributed, and formatted for the referring provider, program sponsor, and payer. Transition to a maintenance protocol built around what worked for this specific patient.
The program does not end at week 16. What ends is the intensive intervention phase. What follows is a maintenance protocol built around the behaviors and clinical changes that produced results — designed to hold them and continue building. The care team transitions with the patient.
